There is also a downloadable version of the form that you can e-mail, fax, or mail back to me, and I will set up an account for you. Also, please have all previous medical records e-mailed or faxed to me.

If your companion animal is truly a family member, than I will provide basic care at your home including:
When your pet is relaxed in his or her own home environmental influences of medical and behavioral conditions can be better assessed and a more accurate assessment of their overall health is obtained. You will have the time you need to discuss your companion’s health and have your questions answered without the rush, noise, and stress of the hospital setting. Species covered:
*All companions must be available to examine at the time of your appointment. *Legal guardian of pet at least 18 years of age, must be present at the time of exam. *This service is for routine exams and minor medical conditions only. *Patients may need to be referred to a hospital and or specialist for further diagnostics and treatments. *Medical emergencies should be seen immediately by a full service veterinary hospital. *I DO NOT PERFORM HOME EUTHANASIA UNLESS ALREADY A ROUTINE PATIENT AND ALL TEST AND TREATMENT OPTIONS HAVE BEEN ATTEMPTED . |
Pricing |
A check. cash payment, or online payment through PayPal is expected at the time of service.
Call me for a specific quote but visit and exam prices are as follows: House Call: $65.00 + $ 0.67 per mile. A single fee to come to your home. Wellness Exam: $68.00 per pet. Recheck Exam: $48.00 per pet. Acute Exam: $95.00 per pet. For non-life threatening conditions that need a same day visit. Emergency Visit: $135.00 per pet. For stabilization of immediately life threatening conditions. Patient will likely require hospitalization and advanced diagnostics and treatments. Service only within 5 miles of South Glastonbury center due to likely need for hospitalization and delay of emergency medicine, diagnostics, and surgery. House call fee and mileage charge will still be charged if pet is not available at time of service or if pet is too aggressive to be examined. Payment is due in full at the time of service.
